We offer our services across the continuum of health - from wellness to illness and back again. While we offer a personalized service that is customized for each client, a general description of our services follows.
+ Make regular visits to assess quality of care
+ Provide advocacy and support during emergency hospitalizations
+ Act as a liaison with healthcare providers to determine condition, treatment options and discharge planning
+ Ensure that relevant past medical history is conferred to providers, avoiding errors of omitted medications or therapies
+ Eliminate costly duplication of tests and procedures by maintaining an accurate and confidential client database
+ Follow-up meeting/conference with client with emphasis on achieving client understanding
+ Make regular visits to assess quality of care
+ Communicate with staff in order to achieve optimal outcome
+ Monitor for violations of resident’s rights or dignity
+ Ensure that necessary services to maintain resident’s physical and mental health are maintained
+ Assess for poor quality of care, including inadequate personal hygiene and slow response to requests for assistance
+ Verify no inappropriate use of chemical or physical restraints
+ Address any resident concern about quality of care or quality of life
+ Assist with management of complex health issues, including polypharmacy
+ Assess home environment with an emphasis on safety
+ Make visits to the home to determine competency of home-care providers
+ Assess new or worsening symptoms and help with your decision whether or not to visit the doctor
+ Prepare for and assist with physician appointments
+ Provide education and preparation for planned hospitalizations
+ Make referrals to community resources to enhance quality of life
